Referral Forms

Please request a Referral and Health Summary from your regular doctor in advance of your appointment.

If you’re unable to get a Referral from your doctor, you can simply call the clinic reception at your doctor’s office and ask that a Health Summary be emailed to info@midispensary.com.au or sent via ReferralNet to MiDispensary.

Please ensure your Health Summary states your diagnosed chronic condition and medications you have trialled.  Please note, we do not need your entire medical history.
